What is a blood type?

A blood type is a classification of blood based on the presence of specific antigens on the surface of red blood cells. There are four main blood types—A, B, AB, and O—which are determined by three alleles of the ABO gene located on chromosome 9. Type A individuals produce the A antigen, while type B individuals produce the B antigen. Type AB individuals produce and express both the A and the B antigens. Type O individuals produce a nonfunctional glycosyltransferase that does not modify the surface proteins at all, so neither the A nor the B antigen is present.

Why does incompatible blood cause a fatal immune reaction?

Type O individuals will produce antibodies directed against A and B antigens when exposed to them. Similarly, type A individuals will produce anti-B antibodies, while people with blood type B will produce anti-A antibodies. When incompatible blood is transfused, the reaction is triggered by host antibodies destroying donor red blood cells, and AHTR (acute hemolytic transfusion reaction) typically occurs when there is an ABO blood group incompatibility, and is most severe when type A donor blood is given to a type O recipient. The most severe reactions involve intravascular hemolysis; the donor red blood cells are destroyed by the recipient's antibodies while they are still inside blood vessels, through antibodies that strongly activate complement, which in turn lyses the donor red blood cells, and hemoglobin is released into the plasma and excreted in urine, turning the urine a dark brown color.

What is the Rh factor?

In 1937, Karl Landsteiner and Alexander Weiner discovered a new blood type: the rhesus blood type, or Rh factor. The rhesus protein is named for the rhesus monkey, which also carries the gene, and is a protein that lives on the surface of the red blood cells. This protein is also often called the D antigen. If your blood type is negative, then your blood cells lack the Rh protein. Those with Rh negative factor will produce the antibodies, therefore someone with Rh+ blood can receive both Rh+ and Rh- transfusions, but those with Rh- can only receive Rh- blood.

Why is O negative the universal donor and AB positive the universal recipient?

O-negative blood type holds the rare distinction of being the universal donor, making it a crucial asset in emergency medicine. O-negative blood is the only blood type that lacks A and B antigens and has no Rh factor within the ABO blood group system. This means it is compatible with all blood types and is least likely to cause a transfusion reaction or Rh incompatibility when transfused.

AB-positive (AB+) blood type is the universal recipient. A person with type AB-positive blood can get a transfusion of any blood type. Your blood recognizes all potential antigens as safe, so your immune system doesn't launch an attack.

How did Landsteiner's 1901 discovery transform transfusion medicine?

Karl Landsteiner was an Austrian-American immunologist and pathologist who discovered the ABO blood group system in 1901. This discovery made it possible to determine blood types and conduct safe blood transfusions. Blood transfusion had involved serious risks and not infrequently resulted in the death of the patient, and therapeutic application of blood transfusion had been almost entirely given up by the time of Landsteiner's discovery. Landsteiner found that blood contains antigens and antibodies that cause red blood cells to clump together when different blood types mix. He identified three groups—A, B, and O—based on their reactions. His identification of the ABO blood group system in 1901 marked a pivotal advancement that transformed blood transfusions from a risky procedure into a safe and standard practice, significantly reducing the incidence of transfusion reactions.

Why does Rh incompatibility between mother and fetus require medical intervention?

Rh isoimmunization occurs when the mother produces antibodies to Rh-positive fetal red blood cells during pregnancy. The destruction of fetal red blood cells can lead to anemia or death in a fetus or newborn. Typically, this is not a concern for a live birth with a first pregnancy, but it poses a greater risk in later pregnancies because the mother develops antibodies to attack Rh-positive blood types in future children.

Rh incompatibility can be treated with a medicine called RhoGAM, or Rho(D) immune globulin, which acts like a vaccine. Women who are Rh-negative generally get a dose at 28 weeks of pregnancy and again within 72 hours of labor. The two-dose strategy reduced the rate of isoimmunization from about 15% to 0.2% in at-risk pregnancies.
